Adobe Sign initials changed to check marks

I signed a multi-person document using Adobe Sign. Every time I clicked my assigned boxes, my initials were shown. Once I finished signing and sent the document to the next person, my initials had become a check mark.

How did this happen and how do we make sure it never happens again?
