Adobe Sign not picking up columns from CSV FIle

Hi I have created a 4 field CSV file - Email, Company Name, ABN and Company Address so that I can Mega Sign a Confidentiality agreement to 300+ companies.

I call each field:

_es_signer_email

_es_signer_companyname

_es_signer_companyABN

_es_signer_companyaddress

When I upload into Adobe sign, I tick the box to adjust the fields and then it processes the document. Only the Company Name field appears in the signer fields on the RHS to choose form, so ABN, Address cannot be automatically be imported into the Confidentiality agreement.
