Adobe Sign sent a document to a different person

As per my subject line. I bulk sent a document to multiple entities and there is one transaction so far that did not send it to it's intended person. It was sent to a different entity from another organization despite that person not in my email list. I tried contacting Adobe Support but it is not working and just keeps redirecting me back to the home menu.

 

I need asstance to figure out why/how. These two entities are not the same.

Did you enter the email addresses manually in the bulk send page or did you do it using a CSV file?

It was a CSV file. The one that signed the document was nowhere in my csv file. The document in question was sent to company A but a person from company B (who is not in my list) signed it. I don't know how that is possible. I even checked the audit log and the email address was from company A but the signee was a different name who was from company B.
