AdobeSign Name field is showing as blank

I am sending out an AdobeSign template that has a name field, text field, signature field, and date fields. I have sent it out to multiple signers, although one signer is unable to populate the name field. It is showing up as blank, but there is a name field included in the template. How do I resolve this issue?
