Agreement status shows "cancelled" after sending out for signature

After I put in the signature field and submit, the agreement appears in the document portal but the status is "cancelled". tried a few times with different email but same results. There is no email sent to my email nor the cc email nor the recipient.

I have sent out successfully previously. 

Only started happening today.What could be the cause?
