agreement to sharepoint multiple filed mapping

Hi,

I have a requirement to use agreement field to set the document title in SharePoint list. I want to use multiple filed from the agreement template to set the title in SharePoint. In below image, I want to use multiple fields from template concatenated with "-". Is it possible? if not any alternate solution suggestion?
