Autofilling items in the wrong locations

I have uploaded multiple forms I need to have signed by multiple people.  When I send it out and begin completing the forms some of the fields from the previous forms are auto-filling in next forms when they should not.  How do I fix this.  I have gone back and replaced all the field by using a new text field but this has not solved the problem.  Please help!
