Automatically Include Printed Version and Date for "Biometric" Signatures

Feature request: don't force us to choose between having the printed version and date when using a hand-drawn signature. Either allow the user to manually type name/initials after doing a "real" signature, or use some kind of OCR. And add the date regardless.
