Building a continuous approval by signature form

I'm trying to create a single document that can be signed multiple times on different dates by the same people. It's meant to be an approval form for money transfers that auditors can refer to as a single document instead of lots of individually signed ones. Right now, all I have is the first line and I know that if I send it to sign, then it will no longer be usable for future entries. Any ideas? Thanks!

If you need information and a signature filled by the same recipient on the same form, you can do this using the Acrobat Sign Workflow feature. For more information on this feature, refer to the following help document: https://adobe.ly/3WTCWXd
