Bulk Sign with Secondary Signer

Bulk Signing only support a single participant at the moment. Howver you can use the GigaSign tool which does allow for multiple participants per agreement

As you are sending the document for signature using the Send in bulk option, it is not possible to add additional recipients in the form. 

As you need to get the signature from individuals and need a second signature from supervisor, you may use the webform. For more information about the web from, please refer to the following help document: https://helpx.adobe.com/sign/using/web-forms.html.
