Bulk upload to multi-line, multi-signature form

We have a timesheet form with multiple lines for the my team to sign.

The layout similar to:

Name | Time in | Time out | Signature

Is there a way to upload a .csv with the information and have it populate a single form to send. I don't want to accumulate the timesheet across multiple form as we have to send the complete sheet in each week.
