Can't create a template as a form fillable layer?

I'm following the steps in the tutorial to create a reusable template, but I don't have the options box the tutorial video shows on the right part of the screen to choose between document, form fillable or both. My only option seems to be creating a reusable document. How can I find an options menu to let me switch to form fillable template?

Hi Allison,

 

If you do not get the options, it seems that you are using the Adobe Sign individual service.

The options are not available in the individual service plan. You can only create the reusable template in this account.
