CFR Part 11 industry-specific compliance requirements

Hello,

I was hoping someone could let me know if I am using adobe sign correctly for a clinical survey.  I want to make sure that the document I set up is configured correctly to meet the CFR Part 11 requirement.  Here is a link to my clinical survey http://bit.ly/2ieP0Oa 

I could not find a way to email Adobe Sign direct to ask them this but if anyone has their email or a way to contact them that would also be greatly appreciated!
