Checkmarks for Checkboxes are Removed when Requesting Signatures

I have checked boxes in filled forms that I need to get signatures for, but everytime I click request signatures and prepare the document, it either unchecks the boxes or inserts new checkboxes. It does not remove already filled in text nor radio boxes.  I've tried this on the website, adobe.acrobat.com too and that is not a solution because it routinely crashes or cannot finish creating the documents. Any advice on how I can fix this issue in the desktop app?

I am having the same issue and Adobe customer services don't want to know or help.... This is extremely frustrating!!

  1. log in to acrobat (acrobat.adobe.com)
  2. click on the very Top Right corner profile icon to get a dropdown.
  3. on the dropdown click on settings
  4. this should open up a popover settings menu
  5. There is a heading "E-signing settings"
    UNCHECK the box that says "Request e-signatures [] Use the latest version of Request e-signatures.

In the meantime, we suggest using the workaround. Add the Checkboxes in Acrobat Sign.
