clients can't fill in forms--entries getting overwritten and changed

My clients are saying responses are getting pre-populated into the wrong areas and the form is changing some of the input they've already filled out.  What could be wrong?

This started happening about 3-4 weeks ago.  Never had this problem before, but now every client is having the same problem.

Hello Gina,

The main reason for information getting auto-populated is form fields having exact same field name. Are you using different templates together to send them out for signature? I would recommend to check the form fields in question and change their name to distinct one.

Thanks Rijul,I did re-do my forms, and I used the "copy" feature to copy one text box to other fields throughout the form.  I thought this was an easy way to just duplicate size of the text field.   But maybe you're saying that in doing this, the same info is being populated and is problematic for the form?

Can I send you an attachment to show you what is happening?   I don't want to attach it here as I am not sure if this will get posted directly to any forums.  But maybe if you can see where the data keeps overwriting and duplicating, we could figure this out for sure.

Let me know and I'll send the encrypted form to you so we can look at it and pinpoint the problem exactly so I can fix it.
