Rather than manually sending a contract for signature, is it possible to have someone fill out a form on my website, have their name and email address pushed to a contract template, and have them complete their signature directly online?

Yes, you can use the create widget feature and upload the PDf form on your website.
Please refer this help document Create Widgets for more information on this feature.
However, the feature is only available with Business and Enterprise service plans.
