Create PDF that clients can fill & sign and send back?

Hello everyone, 
I'm pretty new to this PDF world.. 
I run a fitness business, and want to send clients forms to complete & sign before they start their training with us and then send them back to us?

Is this possible? If so, how do I go about doing this?
