Creating Document Template

Is there a way to create a template and use it for unique but similar documents? 

Example: I have a plea form, signatures and check boxes are in the same spot. But the defendant name and case information is different each time. 

How do I create a template for that? I created a template, but it pulled the actual document with the last person's info.
