Creating optional initial fields

Question

How could I create initial fields where, when one initial field is completed, the other one no longer needs to be? For example, if they choose one option, they put their initials next to that option, and the next initial field will disappear for the other option. I would like to make it where they can only enter initials on one option.

You may add the conditions in the form fields when adding them to the document.
