Custom Signature Block for Various Documents

I need repeated sign offs of documents with the exact same signture and data fields. 

There are hundreds of documents, that follow the exact same syntax. Is there a way to 
define the fields below as a standard set of fields and add the complete set with one interaction
rather than dragging 6 fields into the document every time?

Prepared: Signature 1

Date prepared: Date 1

Reviewed: Signature 2

Reviewed date: Date 2

Reviewed: Signature 3

Reviewed date: Date 3
