Data Field Issue

I am having an issue where I need to put a field on a PDF Document to allow the person to type in number and info for a credit card.  There are several lines on the document that I have been placing individual Data Field boxes and when the client is filling them out he will type in ONE and they ALL fill out with the same info.  They cannot be filled in individually.  So if he fills in his Address for example on the address line and then clicks next to fill out his credit card number...anything he does in the Credit Card field will change all the other fields both above and below it.

I hate to have to go to DocuSign but I need this to work...any ideas?
