Do I HAVE to have a SIGNATURE field included or can I use other field types to use this as a "form"?

When you send the document for signature via Acrobat Sign, you must add the signature field.

If you do not enter the signature field on the document, it is automatically added at the end of the document while sending.

This is the designed behavior.
