Dynamics 365 AdobeSignPlugins.Activities.AddDocument Issue

Hi,

I'm using the Dynamic 365 Adobe Sign managed solution to send Adobe Sign documents from Dynamics 365.  When I use the AddDocument activity in AdobeSignPlugins, it fails to find the form that I attached to the notes but the activity step is still successful.  Is there anyway to trouble-shoot this?  I've attached a screenshot of the D365 system job showing it was successful. 

I won't know until runtime what templates I want to include in the form. Is there an alternative to using AddDocument activity in AdobeSignPlugins?  I'm proficient at writing D365 C# plugins and custom workflow activities
