Error uploading attachment in attachment field

I have put attachement field on document to have signer attach documents, everytime signer tries to attached document it gives an upload error, just saying an error occurred.

According to transaction limits, the files being attached aren't too big.  Is there a reason this upload error would keep happening?  I have yet to have a successful upload.
