Form field template: drop down option selection applies to all documents

Using a form field template with drop-down option for Sender (prefill). Any selected option applies to all downloaded documents. I need to be able to select different options for different documents e.g. option1 to document 1, option 3 to doc 2 ..etc.

How to do this?
