Google Apps HIPAA Compliance form

Anyone a Google Apps Admin and experiencing an issue being able to sign the BAA form to complete HIPAA Compliance in Google for Work?  I follow all steps, in order, but cannot sign the form with either method option.  I took the following pics with my phone, screen capturing wouldn't catch the "no" circle in the Type text box, and the second image doesn't show well that the Apply button is still grayed out after drawing a signature.  I have already reached out to Google but they had no other reports of this issue and couldn't provide any further assistance, even after replicating the issue.  Lastly, this is not a computer/browser/firewall/network related issue at all, I've tried on multiple devices in multiple locations, exact same issue each time.  This is extremely important for my client to get resolved ASAP. Thanks!

- Sender;s email address

- Signer's email address

- The document is accessed through the Google Apps support page here: HIPAA Compliance with Google Apps - Google Apps Administrator Help

- Document link: E-sign

- There is no sender address, at least made known to me as the end user, and the signer address does not matter as anything you enter produces the same result.

- No specific time, this has been ongoing for weeks now

- I have assisted clients to complete this form multiple times in the past year with no issue
