Google Apps HIPAA Compliance form

Anyone a Google Apps Admin and experiencing an issue being able to sign the BAA form to complete HIPAA Compliance in Google for Work?  I follow all steps, in order, but cannot sign the form with either method option.  I took the following pics with my phone, screen capturing wouldn't catch the "no" circle in the Type text box, and the second image doesn't show well that the Apply button is still grayed out after drawing a signature.  I have already reached out to Google but they had no other reports of this issue and couldn't provide any further assistance, even after replicating the issue.  Lastly, this is not a computer/browser/firewall/network related issue at all, I've tried on multiple devices in multiple locations, exact same issue each time.  This is extremely important for my client to get resolved ASAP.
