Group Radio Option Value disappear after converted to Adobe Sign

I am making a template for user. I set a group of radio option for user select. After user fill and sign, the form is fine with non-editable and showing the radio option value correctly. But when click the Adobe Sign to assign the manager sign location, it will process to upload the form as a field form and some Group of the option value has disappeared.
