How do I create a PDF document on my webpage that need to be signed and emailed when completed

Can I create a Terms and Condition Document that is set up on my webpage, I would need it to open up in a new window, so that the user can sign and fill in their details and once they click send it automatically sends a copy to them and to me?

Is this possible?
