how do i get rid of the 2nd signature line with an email requested line under that off the form after its created into a widget

So I have created a widget or Hosted Form, and after completing it to have only one required Signature, it puts another section, or area below that asks for another signature, and email as a required section which i do not want to have there. It creates a whole other page as well because my original signature line is at the bottom.

So,..How do i get rid of the 2nd signature line with an email requested line under that off the form after its created into a widget
