how to create a web form that can be signed and submitted as a PDF via email

I have a client that wants to put 4 or 5 forms on their WordPress website that residents can complete, sign and submit as a PDF via email from the website.

 

Can this be done in any way using Adobe Acrobat Sign or will they need something like Docusign instead?

 

Trying to find the most effective and cost-efficient way for them to received signed copies of their online forms. The forms must be signed, and the old "download, print, sign and mail" process just isn't working for them anymore.

Yes, it can be done.   Create a web form (adobe.com)
