How to make a response to one of four questions mandatory when fields are different

How do I make one the template require and answer to one of these options when one is a text field and the other three are radio buttons?

Any help would be greatly appreciated!

If you are looking for the text field to be optional with radio buttons, it is not possible. You may try creating the first option as the radio button, which once selected, you get the text box to enter the information. You can use field conditions for that (https://adobe.ly/4agfFmd).
