HTML Form to Fill PDF Template and send for signature

I am new to API & E-Sign platforms and am looking to create an online html to collect user information. This form will then post the data into adobe sign to fill a PDF template. This will then need to be issued to the end user for them to ESign.

Can someone please assist / point me in the right direction of how to complete this?
