Integrate may not be the right word. What I am wanting for the business I work for is this:

 

Our therapists will be using a template to do evaluations. I want them to have several options to choose from say under a specific category. When they choose that specific option, I would like it to then integrate into the summary of the patient above. Does that make sense? The other options can disappear, no longer be present. But example:

 

Summary: Patient presented today ready and able to work. 

 

option 1: (patient) presents with heavy breathing and swallowing concerns

Option 2: (patient) presents with smelly breath

Option 3: (patient) presents with funky toe smells

 

Then the therapist could select any of the options given to them, it would integrate to the above summary-and the remaining options would disappear.

 

Please help me!

    Thank you for reaching out, and sorry about the delay in response.

     

    In this case, you may add a drop-down to select the option, and then the user can fill out the rest of the form. However, if you are referring to that, depending on the first selection, the rest of the document will be different for each category, then you may check the Workflow feature in Acrobat Sign.
