Issue with CSV uploading - Mega Sign

Hi!

Im having trouble when trying to upload the .csv file in mega sign. I kept the first row identical as the sample and added the data below. When uploading it shows me error. 

I reviewed a similar post but didn´t help me. 

Additionally, I would like to incorporate different columns than the ones existing (title, company, etc), is it possible?
