Issues with ESIGN field alignment

Question

Image is pretty self explanatory. Some fields loading flawlessly, others wildly incorrect.

Form was working perfect less than four weeks ago. Support are unable to fix - rending the software useless.

Any help from community greatly appreciated.

(legacy & new forms same issue)

Please let us know what you mean by fields is widely incorrect. Are you referring to the information in the Form fields, or is the Form field type incorrect?

Are you using a template form or uploading the document using Acrobat Sign? How was the form initially created? Did you make any recent changes that may have resulted in this behavior?

If you are uploading the form, please share the form so we can check.
