Missing description in Form

We use Adobe sign to send out applications.

Some lines include a description of what to enter in the field using a Custom Format script. eg: Enter Classification Here.

When we send it out, that description does not appear in the field. Is there any way to include that or do we need to enter that description as a Default in the form in Adobe Sign every time we send it out?
