Would you mind sharing the screenshot of the window where the name is pre-populated?

Share the steps on how the document was delegated both times.

Check if you can replicate the behavior with other forms. Try creating a test agreement and use the same steps.

Share the screen recording with us if it occurs again with a new form. We will get this checked.
