Need help no docs coming back and patients are losing their information..I have submitted this and keep getting error, please someone help us get this corrected

Hello???  Is anyone available to fix this problem.  We are losing docs and they aren't coming back.   Patients go through 20 pages to fill out questionnaire and then it goes blank.  I have very frustrated customers.  Please help us.  Thank you.
