Need multiple choice option for signature/initials

I have a form where the customer has a choice to make.  If I put signature/initial fields at every choice it requires all fields to be completed and they can't change them to "no".  Is there a way to send a document for signature that is "multiple choice".

i.e.

I want "A" signuture field

I want "B" signature field

I want 1 initials or 2 initials or 3 initials

We want the customer to be able to select A or B along with 1, 2, or 3
