Non Profit YMCA - Adobe Sign for Waiver Forms

Hi,

I am from the Summit Area YMCA, and I have a 15 day trial right now with Adobe Sign.  We are looking for the best way to set Adobe Sign up to allow us to:

  1. Create reusable fill-able forms (Membership Waiver, Termination, On Hold Request, Change of Status, and so on) that a member can easily get to via a KIOSK/Website to sign after they complete their membership details in our member database.
  2. Save the signed forms under their name or ID (Unit ID automatically generated by our system).
  3. Download the saved forms to upload to our 3rd party repository called Content Verse.

I just tried a test widget with a test waiver form, and it had me verify my email address, which we don't want.

  1. How do we setup the reusable fill-able forms, so that no verification is needed to sign?
  2. I know we can setup the widget URL on an iPad/KIOSK/Website, but is there any other ways to do this?

We basically just need the member to be able to sign the forms while present, after they complete their membership in our database, with no verification required.

Hi, 

I also work for a YMCA and am trying to sort out a digital solution for capturing waivers.  The process above is exactly what we're trying to accomplish as well.  We have the full Creative Suite and have just started poking around the e-signature portions. 

Please let me know what may be the best way to move forward.
