One pdf signed by multiple users over multi-year timeline

Looking to setup a PDF that gets signed periodically over a multi-year timeline. When a team member joins the group, they need to sign the PDF and the superviser needs to sign the PDF to confirm. When a team member leaves the group the superviser needs to sign a 2nd time to confirm the date that the member left. Is this possible without loosing the digital signatures.
