Outputted PDF glitched with words over the top of other words

Im not sure why but the output of my pdf has glitched.

It has words over the top of other words and looks very unprofessional. My test looked fine of course, but as soon as i sent my contract to a real client to sign it glitched. Great. Anyone else having this issue? Am thinking of re-uploading the form, but its taken me so long to make.

    Could you please share the screenshot of the output so that we can diagnose the issue correctly.

    For privacy reasons I've cut off the names.
    My contract has been doing this sort of thing.
    There are not fields on top of fields, I've double checked this.

    Seems there there is a field over the data or there are duplicate fields placed on the form, please fix your form.

    Remove the duplicate fields to input the information .
