pb with insert a document for sign in my library

the document is already a form and when i would like to pu in library for send at some client it doesnt reconize the field

i don't understand

what can i do

Seems that the documents is not prepared correctly or it is not prepared using the compatible application.

Please upload the document as a Library template & then place the fields on the document in the Adobe Sign Environment.
