Radio buttons get reset when uploading form to request e-signature

I have a fillable PDF including some text fields and radio buttons. After filling it and adding my own signature, I uploaded it using the 'Fill & Sign' feature in order to request my supervisor's signature. However, the radio buttons that I had set in the original form, were cleared in the uploaded version. I have tried several times but to no avail.

Can someone suggest a fix?
