Report codes: Causes and solutions?

I used Acrobat DC to create an interactive form that uses
radio buttons, text fields, check boxes, date fields, and action fields. When I
go to e-sign, the review window shows report codes. What is causing report code
1000 (doc contains hidden behavior), 1001 (comment or form field may silently
change), and 1008 (doc contains hidden behavior)? How do I get rid of these? We
don’t want a document that contains these report codes to end up in court and raise
questions about the validity of the signature and document.
