Reusable Form Field Layer

How can I re-use a template with the same field layers on different documents? I created the template on a 2 page document. However, when I try to apply this same template on anything more than 2 pages, it completely messes up the format.

Hi there,

 

I have already done this and created the template. However, when I apply it to different documents the format is completely messed up.

 

I want to be able to populate the signature, name, title, and date field at the last page of each document without having to do this process manually. I thought creating a field layer template would fix this but the format seems to mess up each time.
