Rogue signature

Hello,
I sent a document out for my client to sign from a templet, (I sent several of the same templet to different people within the same hour of each other), and for one of my clients she said a name was already in the signature field when there shouldn't have been. She submitted it, and when I looked at it the name is there but there is no time stamp of when it was recorded and it is popping up as a link, which I will not open. What would cause this? No other people had this issue.

If you are referring to the blue line with the name under the signature, please refer to the following help document: https://helpx.adobe.com/sign/kb/to-enable-disable-name-date-below-the-signature.html

Try sending the test document to the user again and see if you get the same after changing the settings.

The signature I am referring to does not have a blue line under it, my client said when she opened the agreement I sent her that there was a signature already on the line and that she could not edit it. It was black and did not have a time stamp on when it got there...and no other lines had been filled in. 

It states the owner name as "Natalie Rodriguez" and my client said when she opened it it already was signed. I do not even know anyone with that name so I am not sure how it showed up there. This was from a template I have sent many times and this has never happened before. I included the screenshot below, You can see where the initials of my client was accepted and under where it says owner and

when it got there...and no other lines had been filled in.
