ServiceNow Adobe Sign - No Document Fields

I recently installed version2 of the Adobe Sign for ServiceNow app,and I cannot get the document fields from a Library Template. I only get 1 Signature Field no matter the template.

I also cannot use the Local Template feature as it comes back with an error when try to post the template to Adobe Sign.
