Signature doesn't save

I've recently had multiple recipients tell me they've signed a document, sometimes more than once, but I'm not getting a the signed copy and when I check online it shows that they have viewed the document but not signed it. This had happened with different documents and three different people who have succesfully signed many other documents. 

Any idea what might be happening and what I can do to fix it?
