Signature Fields Disappearing

My current process for getting a document signed is to upload a file in Adobe Sign and assign different signature fields throughout the document before signing. I'm using adobe sign for a few years and never had issues until now.

 

In the past week or so, when a document has been sent out, the fields that I have inserted have been removed and there is just one global signature at the bottom of the document. 

 

Also there is always a message saying :There are recipients who need to sign this document that do not have a signature field assigned to them. You can go back and add signature fields or we will add a signature block for those recipients. Do you want to review or send the document?

    Are you still experincing this issue?

    If yes, would you mind sharing the video caturing the same? It will help us to undertsand the issue better.

    Did you check if this occurs with all the documents you send for signature?
